Mandatory Daily Health Declaration

Mandatory Daily Health Declaration Process:

It is the responsibility of all parents and guardians to ensure that they undertake the Mandatory Health Declaration Process EVERY DAY before allowing ANY McMath student to come to school.  The poster showing the requirements is attached for your reference.

We would suggest that parents and guardians ensure that they have this this document easily accessible in their homes and remind them that they MUST undertake this self-assessment for each student in their care daily and MUST NOT allow the student to attend school if they are exhibiting any symptoms of illness. Adherence to this process includes direct consultation with health officials via calling 8-1-1 to confirm what actions may be required before an employee can return to work in addition to being symptom free. 

This is a crucially important process and no exceptions will occur for ANY of our students.  As much as we are focusing on the health and safety procedures of cleaning and disinfecting, learning cohorts, handwashing and respiratory etiquette, just as crucial is staying home when sick and not introducing illness into our schools.
