NOTICE OF LATE RETURN FORM – 2023/24

(Due to Extenuating Circumstances)

Families with students who will not be returning to school by noon on Wednesday, September 6th 2023 are asked to complete the attached Notice of Late Return Form ) and submit it to the school's office or via email at mcmath@sd38.bc.ca.

Please note that students must return by September 13th 2023 to ensure they retain their spot at McMath Secondary.

This form needs to be completed and returned to the school by JUNE 24 2023. *Please note: Both English and Chinese versions are noted in the attachment.*
